Your card is a LR6611 OEM variant.

i have poked the cx88xx module a bit more and i got it working ( so far )
with te following line :
modprobe cx88xx i2c_scan=1 card=5 tuner=63 && modprobe cx8800

dmesg tells me this

tuner=63 is wrong, you have FM1216ME MK3 (and not FMD1216ME MK3). Try with correct tuner.
