Adding Attachments to a PDF

Hi,
I want to add one or more attachments to an existing PDF file programmatically.
Could anybody who have succeeded in doing that just write the 5-10 lines needed?
This would be *very* much appreciated.
BTW, this topic has been brought up before and seems worth an entry in the tutorial.
