PushPutton Appearances using pdf pages as face of appearances???

Is there an example of how to read an existing page from a pdf and create a PdfTemplate so the page could be used as a face on PushButton using the PUSHButtonField object?

We have managed to do this with jpg and png images but we need to use 3 different pages from inside a pdf as the three different appearance faces of a push button?
